Forms: Public Service Loan Forgiveness, DoDEA Form 5013, and Heartland/Perkins Deferment
District employees may obtain employment and payroll verification through Experian Verify. This service offers secure, confidential internet access and telephone support Mon-Fri 9am – 7pm (ET).
To access Employment Verification Information, verifiers will need to register and submit their requests online at www.experianverify.com. They will need the following information:
No company code or salary pin required. If your verifier has questions or encounter issues, they can contact customer support by calling (404) 382-5400, option 2.
Do you have a Public Service Loan Forgiveness form? You can submit your request to Experian Verify by fax at (404) 829-1336 or by email to https://voie.experian.com/app/ee/eeregister.aspx. Select “create an employee account” and enter the following:
Login with your Credentials by following prompts: A password setup link will be sent to your email address. Click the link to create a password and activate your account. Once logged in select the “Letters” on homepage or sidebar menu. Select “PSLF Letter”. A letter with submittal instructions and completed ECF will be generated.
Do you have a Social Service Request from a government agency? Please submit your request to Experian Verify by fax at (404) 829-1336 or by email to verify@experian.com. You can also have your caseworker send the request directly to this fax or email. Be sure to include the following information on the form:
Requests will be processed within one (1) business day. If you have any questions, you can contact Experian Verify’s social service department at (404) 382-5400, option 4.
Click Create an Employee Account. A password setup link will be sent to your email address. Click the link to create a password and activate your account. Lastly log in with your credentials.
