Nevada does not offer reciprocity on licenses from other states. It is the applicant's responsibility to obtain the appropriate license. The state office requires (a) official transcript(s) with the university seal and degree(s) posted -- see your registrar, (b) results of a recent TB skin test or X-ray, (c) a fingerprint authorization statement, (d) fingerprinting. The packet mailed to you from the state must be completed as a packet and all items (a-d) returned simultaneously to the state along with the standard fee.
